Employer Notification Requirements for COBRA Coverage:

Your employer must notify the contract administrator no later than 31 days after the following qualifying events:

 

1. Member’s death;

 

2. Member’s termination of employment or reduction of hours;

 

3. Member’s entitlement to Medicare;

 

4. Employer’s bankruptcy, if member has retired.
