Employee’s Notice of Qualifying Event

You or a Qualified Beneficiary or authorized representative must notify the Contract Administrator no later than 60 days after the date of the following qualifying events:

 

  1. Divorce/legal separation;

 

  1. Dependent child ceases to be a dependent child;

 

  1. Occurrence of a second qualifying event after you or a qualified beneficiary has become entitled to COBRA;

 

  1. Determination by the Social Security Administration that you or a qualified beneficiary is disabled.

 

If notice is not given in a timely manner, the right to continuation will be lost.
